CLINICAL NUTRITION - FOOD & NUTRITION SERVICES

Clinical Inpatient Dietitian Services 

The WDMH Clinical Dietitian:

  • Assists with inpatient nutrition risk screening program for timely identification of patients at risk for protein-calorie malnutrition
  • Performs SGA and/or Comprehensive nutrition assessments for patients identified as at risk for malnutrition or presenting with nutrition-related illness
  • Determines patient nutrition status and nutrition-related diagnosis
  • Collaborates with the individual/family and health care team
  • Establishes and coordinates implementation of a nutrition plan of care, consistent with the overall plan of care, to meet identified needs
  • Monitors outcomes and achievement of nutrition goals, re-assessing and revising plan of care to optimize outcomes.
  • Provides nutrition education and/or referral to appropriate community nutrition resources
  • Assist Food services through menu modification based on special dietary requirements, including food allergies/intolerances

Nutrition care plans are outcome-focused and incorporate evidence-based best practice. Plans are formulated to optimize both clinical and fiscal outcomes and emphasize health promotion and disease prevention. As part of patient care, the Clinical Dietitian is responsible for educating patients/caregivers about their overall disease and especially how it relates to nutritional management. The Clinical Dietitian is part of, and promotes, a collaborative health care team approach to patient care.

Clinical Ambulatory Dietitian Services

WDMH Dietitians are trained to advise you on food, diet and nutrition. In every province, practicing Dietitians are licensed, so you can be assured you are getting safe and ethical advice.

  • The WDMH Dietitian can help you manage medical conditions that may benefit from diet change.
  • Individual Nutrition Counseling is available to provide nutrition assessment and intervention for various conditions such as IBD, IBS, liver disease, chronic kidney disease, celiac, cancer, complicated Obesity/Metabolic Syndrome, known food allergies/intolerance, malnutrition/underweight.
  • Group Education is available for Heart Healthy Nutrition/Cholesterol Lowering, which runs first Thursday of each month, except Jan.
  • The WDMH Dietitian also provides nutrition intervention within TOH Satellite Units for Hemodialysis and Chemotherapy treatments.

Unless otherwise indicated, a Referral for Outpatient Clinical Nutrition Counseling must be completed by a physician and faxed to 613-774-7236 so an appointment can be scheduled through the Dietitian’s office.
