Social Work
The
social worker can help you and your family understand and learn to cope
with your illness, diagnosis or disability. She can also assist with
any lifestyle changes that may accompany this change in your health. As a
resource person, the social worker offers a variety of supports and
services such as:
- Assistance with Financial Concerns
- Private/Disability/Government Pension Plans
- Ontario Works/Ontario Disability Support Program
- Income Tax – claiming medical expenses
- Tax Credits
- Assistance with Drug Coverage
- Counselling and Support
- Adjustment to Illness
- Mental Health & Addictions
- Grief and Loss
- Crisis & Risk Assessment
- Links to Community Resources
- Liaison between the patient and community services/resources such as:
- Social/Recreational Supports
- Diagnosis/Disability-Related Resources
- Assistance with Transportation
